PASSPORT CENTER HOURS 9:30 AM - 5:00 PM MONDAY - FRIDAY
The following items are needed to apply for a passport:
- A completed DS-11 Form. We have blank forms available for applicants to complete at the library, but they are also available online. Each applicant can print a blank form and fill it out (in black ink only) before they arrive, which will save the applicant time. Here is the link for the form.
